Admissions / Enrollment
The high schools are open entry. The following list of items will be needed for the student:
- Student Registration Form: Parent/Guardian signature is required for students under the age of 18.
- Photocopy of a certified copy of the pupil’s birth certificate. Or, other reliable proof of identity and age, including the pupil's baptismal certificate, an application for a social security number, or original school registration records and an affidavit explaining the inability to provide a copy of the birth certificate. Or a letter from the authorized representative of an agency having custody of the pupil pursuant to title 8, chapter 2 certifying that the pupil has been placed in the custody of the agency as prescribed by law.
- Photocopy of immunization records or proof of an exemption from immunization. (Not a condition for enrollment, but required for attendance. Homeless students have 5 calendar days after enrollment to provide proof of immunizations.)
- Academic transcripts from all high schools previously attended. (Not a condition for enrollment.) *The high schools will provide a transcript letter request form and request transcripts from prior schools.
- Arizona Department of Education Arizona Residency Documentation Form.
- ECAP - Education and Career Action Plan
- Withdrawal form (school will request from the most recent high school attended; not a condition for enrollment, but is required for attendance)

- Special Education Services: The high schools provide special education services for students with individualized education plans. The high schools ensure that each student with a disability has the services determined by the individualized education plan.
